Our value-added home health care services are enhanced by our specialty programs. Each program is an outcome-driven model of care with stand-alone strength which overlaps and intersects with other programs resulting in a truly holistic approach.
For example, our CHF Program works in conjunction with our Hospital Admission Reduction Program and Home Again Transitional Care Program to equip CHF patients with the tools needed to realize optimal recovery at home and avoid unnecessary hospital stays.
