Heart failure does not mean that your heart has stopped or is about to stop working. It means that your heart cannot pump enough blood (oxygen and nutrients) to meet your body’s needs.
Fortunately, Congestive Heart Failure (CHF) can be treated. We can help patients with CHF by optimizing their transition from the acute setting (hospital or nursing home) to the home setting. Once home, our nurses will work with patients to be sure they understand how to take their medications and best practices for disease management.
